1. Identify each program you are implementing under program activity numbers 107 – 196 of your approved FY2008 SAPP work plan.

• You are required to report on all program types included under program activity numbers 107 - 196. This includes Youth Prevention Education ATOD Curricula, Parent/Family Education Curricula, Mentoring Programs, Tutoring Programs, Communication & Social Norms Campaigns, Technical Assistance to Student Assistance Programs and Technical Assistance to Develop/Enforce Public Policy.
  
• The information entered into the Workplan Program Information screen of OnTrack should provide an up-to-date record of your agency’s approved work plan activities for FY2008. You can access the OnTrack system at http://www2.cprd.uiuc.edu/BSAP-OnTrack/

2. Download and complete the FY2008 Annual Evaluation Progress Report Cover Sheet.

After clicking on the link to the cover sheet, select “save” and save the file to your computer.  Do not select “open” and enter information into the document without first saving the file to your computer.

• All agencies are required to complete Table 1: Agency Contact Information.
• If you are not submitting a process evaluation summary report form when one would be expected, you must provide an explanation for the omission in Table 2: Unreported Program Activities.

3. Download and complete the summary report forms that correspond to each program activity identified in Step 1.

• Parent/Family Education Curriculum--Standards-Based

• Technical Assistance to Develop/Enforce Tobacco and Alcohol Public Policy—Standards-Based

4. Compile and submit your FY2008 Annual Evaluation Progress Report.

Your report should be assembled in the following order:

• FY2008 Annual Evaluation Progress Report Cover Sheet
• Completed process evaluation summary report forms in numerical order

The FY2008 Annual Evaluation Progress Report is due July 25, 2008. Refer to the BCPP SAPP Reporting Requirements for FY2008 for more information on where to send your report.
