The tools below are examples of assessment tools used by communities around the state. They provide you with a starting point for seeing how others have approached community assessment. Feel free to use as few or as many of the tools listed as you find helpful. The tools can be revised to better suit your community’s needs. Each tool can be saved to your computer as a Microsoft Word file and then, edited as necessary. Finally, if you have developed or identified elsewhere a tool that you think others would find helpful, please send a copy to Erin Ficker at at eficker@uiuc.edu and we will add it to the website.

This is truly a resource exchange. Tools are not screened for validity, reliability or other research-oriented requirements. Tools will be screened to assure that there is not a great deal of duplication. The goal is to provide a broad array of tools rather than many different versions of the same type of tool. Please contact the agency listed on each tool for further information.
