Process Evaluation Data Collection Tools

Once you have downloaded the process evaluation summary form that matches your program, review the form and note the types of information that are requested. Then, explore the list of tools below to find those that will assist you in collecting and recording the information you will need to supply in the process evaluation summary form. We encourage you to explore all of the tools listed below. We have attempted to group them into broad categories in order to guide your search but not all of the tools are easily classified and some of them may overlap several categories.

None of the tools listed below are required. The tools are made available to provide you with a starting point for data collection. You may have found or developed your own process data collection tools that are ideally suited for your program. Feel free to use as few or as many of the tools listed as you find helpful. Also, feel free to revise the tools to better suit your needs. It is likely that the tools listed below will be most helpful to you after you have edited them to fit your individual program. Each tool can be saved to your computer as a Microsoft Word file and then, edited as necessary. Finally, if you have developed or identified elsewhere a tool that you think others would find helpful, please send a copy to Trina Ragain at ragain@uiuc.edu and we will add it to the website.
