What should I do? Answer: Do your very best to administer the youth and parent surveys to everyone on your list. If a youth is no longer in the program, make a note of this on your survey list and be sure to return the blank survey to CPRD. If your site is participating in the Benchmark Self-Assessment Process, please collect at least 20 completed youth and parent surveys. If you are participating in the Benchmark Self-Assessment Process and more than 10 youth on your list are no longer in the program, you should call Kay Mulhall at 217-333-3231. Question: Given the names on our survey list, I am almost certain that we won't be able to collect youth and parent surveys from many of these youth. What do you want me to do? Answer: It depends on the reasons why you think you won't be able to collect youth and parent surveys. Question: We are participating in the Benchmark Self-Assessment Process. I am confused by the different survey requirements for the overall evaluation and the benchmark survey requirements. Can you explain? Answer: The survey procedures and the surveys themselves are identical for the benchmark sites and for those sites collecting data for the statewide evaluation. The only distinction is that we must have a minimum of 20 completed youth and parent surveys from the benchmark sites.
